Health Connection App

The Health Connection iOS and web apps help clinicians find programs and services for clients with chronic conditions and health needs.

Health Connection contains programs delivered by public and non-profit agencies, physicians, and private home health providers operating on the North Shore of Metro Vancouver. In time, Health Connection will include more resources for different populations on the North Shore.

Key Features:

1. Find resources nearest you (based on current location)
2. Search by keyword, program name or specialty (e.g., cardiology)
3. Filter by type of program or service
4. View results in a map or list view
5. View essential information, including location, hours of operation, clients served, referral process and fees
6. Email links to program descriptions for easy printing or sharing with colleagues or clients

Health Connection was developed with the North Shore Divisions of Family Practice and Vancouver Coastal Health.
