HealthConnection: A Digital Directory for Community-Based Health Resources in North Shore Vancouver

Connecting Vancouver North Shore Clinicians and Clients to Local Care

The HealthConnection mobile and web apps help healthcare professionals quickly locate health and social services for clients with chronic conditions and complex needs. Built with Vancouver’s North Shore communities’ unique needs, HealthConnection bridges the gap between clinical care and community support.

The directory combines a wide range of programs delivered by public and non-profit agencies, physicians, and private home health providers across the North Shore of Metro Vancouver. These services include peer support and transportation to specialist care and in-home services. HealthConnection also highlights social service agencies that enhance care delivery in the community.

Built in Partnership with Healthcare Leaders

HealthConnection was developed closely with the North Shore Divisions of Family Practice and Vancouver Coastal Health, ensuring alignment with regional healthcare goals and frontline realities. We build the platform using our open-source Anahita.io, an open-source framework for building knowledge-sharing applications. Our company, rmdStudio, is the founder and active maintainer of the Anahita project. We implemented the HealthConnection North Shore Vancouver in partnership with regional healthcare stakeholders.
